Introductory Rates (ATOS)

$85: 60 minute (55 min service)

$45: 30 minute (25 min service)


Regular Rates (ATOS)

$100: 60 minute (55 min service)

$55: 30 minutes (25 minute service)


$150: Medical Massage 60 Minute $150 (PIP & L&I only)


Peak Function Health accepts Cash, HSA, PIP, & L&I.



Insurance Policies


Peak Function Health accepts PIP and L&I claims. We do not accept 3rd party policies.  


For motor vehicle accident (PIP) and L&I claims, we need your active claim number and doctor's prescription at time of service. Massage can not be done without those items.  


Please contact our office vai email at [email protected] and we will send you a HIPPA level secure email that you can send the required documents. Once we have the required documents, we will contact you for scheduling.  


The client is also responsible for any denied claims. A signed agreement acknowledging and confirming this responsibility is required before your first massage appointment.


Peak Function Health does not accept major medical insurance. The reimbursement rates haven't changed since the early 2000's and are way too low to cover expenses today's world.  


We do offer courtesy billing! After cash payment for services, we can bill on your behalf to your insurance company for you to be potentially reimbursed.  


You will need to do some homework pertaining to your policy. You will need to know if you have out of network benefits. Where are you with your deductible? How many visits are covered? Be careful with this. It is common for policies to combine massage therapy, physical therapy, and chiropractic under one benefit, each counting as a visit. You will also need a qualifying provider to provide a diagnosis code even if your policy doesn't require a preauthorization. This is because massage therapists can not diagnose. Every claim requires a diagnosis code.  


Courtesy Billing is a service provided for the client. It is a superbill sent on behalf of the client. Most places that provide superbills do not offer this service. All responsibility pertaining to insurance specifics is that of the client. Follow ups on claim denials are the responsibility of the client. 


A signed agreement acknowledging client responsibility for insurance claims and denials is required before taking advantage of the courtesy billing service. A superbill can also be generated for the client to send in to their insurance company for potential reimbursement.
